Arts & Crafts Fair Application:

Associate Membership Application:

Associate Membership will make you an “official” part of our community, eligible for volunteer leadership positions on committees and the Board of Trustees, as well as voting privileges on certain topics that come before the community during the Annual Meeting. Celebration of Life:
An online form has been developed for people to submit births and deaths for the annual Celebration of Life reading and publication.

Construction Review Committee Building Project Packet:
Thank you for notifying the Congregational Summer Assembly of your plans to build, remodel or rebuild on the CSA grounds. You are only required to obtain CSA approval if you are expanding the “footprint” of your existing structure or if you are modifying your well or septic system. Adding a deck, porch, access ramp, stairs, etc. qualifies as increasing your structure’s footprint.
Education Fund Scholarship Application Form:
The CSA Education Fund was established October 24, 1992 to provide scholarships to CSA and Benzie County students. Scholarship applications are reviewed by the CSA Education Fund Trustees and granted considering the following: financial need, scholastic ability, strong commitment to service within their community, and admission to an undergraduate or post graduate program at an accredited college or university or into a vocational program.

CSA Expense Reimbursement Form:
To be used for all requests for reimbursement for expenses or mileage by staff and volunteers. It is not intended to be used by vendors.
Mooring Request Form:
Registering for a mooring must be completed electronically using the form at the link below. Requests using the form will be handled as they are received, and an invoice will be sent to you promptly after the form is processed.
Owner Renter Registration Form:

Cottage owners must inform renters of their responsibilities to purchase Assembly tickets and must see that fees are paid. Click here for information on registering your renters.
Proxy Form 2024 Annual Meeting:
Lot Owning Members eligible to vote who are unable to attend the Annual Meeting can file a proxy vote with the Secretary at least 24 hours prior to the meeting. Proxies do not count towards a quorum.

Use of CSA Building, Room, or Common Property Agreement
The Congregational Summer Assembly (CSA) allows limited noncommercial use of its buildings, rooms, and common property (“CSA Space”) to persons or organizations that have an association with the CSA. Uses include meetings, memorial services, weddings, concerts, and educational presentations. This agreement between the CSA and “Property User” governs such use. The CSA Affiliate has completed the Request to Use CSA Space, attached as Appendix A, and such request has been granted, subject to the execution of this agreement.
Women’s Association Funding Request Form:
The Women’s Association will consider funding for programs and purchases which enhance opportunities for women and children. To be considered for funding for specific projects, programs and/or purchases for the 2023 Season, please complete the CSA Women's Association Funding Request Form.
Funding requests to the CSA Women's Association will be accepted from October 1st through June 1st of each fiscal year on a rolling basis, pending the availability of funds. For more information, click here.
CSA Youth Citizenship Award Nomination Form:
The Citizenship Award seeks to recognize a youth member of the CSA who models our vision by cultivating integrity, fostering responsibility, promoting inclusivity, and building confidence within themselves and others.
The award is open to anyone who:
- Is between the ages of 14 -21
- Is active within the CSA community, CSA staff are included
- Demonstrates a strong contribution to the CSA
- Is a role model to other youth
- Demonstrates kindness
